Getting Started

I WOULD LIKE TO TRY THE FOLLOWING TRIAL PROGRAM. As per today's date and to the best of my knowledge, I am physically able to participate with the below mentioned programs. I also acknowledge that I should consult my physician before starting and new physical activity and will bring any medical matter to the attention of the Mechelle's Way Tae Kwondo Staff.


I will not use anything I learn at Mechelle's Way Tae Kwondo for any illegal or unjust purposes.
